Your Child's Nutrition

White Pine Elementary and the White Pine STEM Academy offer meals to all students. In an effort to accommodate children with special dietary needs, a signed medical statement is required. If the special dietary condition creates a disability, a physician’s signature is required. For non-disabled participants, a licensed physician, physician’s assistance or a registered nurse must sign the form. Please complete the Special Dietary Needs Form and return it to the Food Service Department.
